Before your first visit, please complete the attached forms and bring them with you. This will allow us to focus more on your needs and introduce you to our exceptional team. Insurance & Financing
Insurance
As a patient, it's important to verify whether our practice is in-network with your insurance plan. We will gladly submit claims to your insurance provider on your behalf. However, you are responsible for any balance remaining after your insurance has made its payment.
Financing
We accept various payment methods including cash, check, debit card, credit card, and CareCredit. Payment is expected at the time of service, but we can arrange for an extended payment deadline if necessary. Please note that if your payment is overdue by 45 days, a monthly service charge of 1.5% will be applied to any outstanding balance until it is fully settled.
